Yeah, I'm not quite sure what's up with John not being in any of those promo photos. JDM is listed as being in the ep, though, so he's gotta be around somewhere!

Don't know who (or what) that girl is, but she certainly looks dead, don't she? She must be a spirit of some kind that appears to Dean, gives him a hint that this "reality" isn't real.
